Sanford Health of Northern Minnesota contracts with many insurance companies. To be sure that your services will be covered, we ask that you contact your insurance plan directly prior to being admitted.

Below are some of the most frequently asked questions about billing and insurance. If you don’t find the answer you are looking for, please contact the Sanford Bemidji Medical Center Business Office at 218-333-5827.

Will Sanford Health contact my insurance for precertification or authorization?
It is recommended that you contact your insurance plan or provider directly if you have any questions or concerns about precertification or preauthorization.

I have multiple health insurance plans. How do I know who to bill?
Often complicated insurance rules apply. Contact each insurance company for help in determining the proper order for billing.

If my insurance doesn't consider Sanford Bemidji Medical Center as a preferred provider, will you bill my insurance anyway?
Sanford bills all insurance carriers. If your insurer does not include our hospital or other facility as a preferred provider, you may be billed for non-covered charges or be responsible for reduced benefits. Please contact your insurance company directly to verify your coverage and/or benefit amount.

What if my insurance company does not authorize or cover the services I need?
You will be responsible for any charges your insurance company does not authorize or cover. If you cannot afford the services, it is recommended that you contact the doctor who ordered the services to discuss alternative treatment options and other possible funding sources.

Why aren't all services covered by my insurance company?
It all depends on the policy you have. Under any plan, there may be services that are not covered because the insurance company may consider them routine or unnecessary. If you disagree with the decision, you should contact your insurance company about filing an appeal for your services to be reconsidered.

What if my claim is denied?
Contact your insurance company for additional assistance or to file an appeal for your services to be reconsidered.  

What if my insurance coverage changes?
Contact our Business Office at 218-333-5827 to update your information and make sure you have a current insurance card on you at your next visit.

What should I do if my insurance sends its payment directly to me?
Any insurance payment for claims that are sent directly to you should be used to pay outstanding charges to Sanford. Patients/guarantors are financially responsible for all outstanding charges. Either deposit the insurance check and send us a personal check, or forward us the insurance check as soon as possible. Our mailing address is:

Why do I still owe a balance if my insurance company has paid?
Based on your insurance plan, you may be responsible for deductibles, co-pays, co-insurances, and fees not covered under your plan.

If I receive services because of an accident at work, will you send the claim to my employer?
Due to confidentiality, we are unable to send bills directly to your employer. However, we will send claims directly to your employer's Workman’s Compensation carrier. We will need their name, address, and a claim number. You may personally take your bill to your employer and work directly with them.

How do I submit my insurance information?
If you still need to provide your health insurance information, contact our Business Office at 218-333-5827.

How do I contact my insurance company?
If you need to contact your insurance company directly, please refer to the back of your insurance card for the customer service phone number or consult your insurance handbook. Many insurance companies are online, below are links to a few.

Does Sanford have a Financial Assistance Program?

How do I Find Out if I Qualify for a Minnesota State Healthcare Program?

For those who qualify, Minnesota has several healthcare programs. To find out more, contact Sanford Bemidji Medical Center's Outreach Services at 218-333-6373 or your county human or social services office or visit the Minnesota Department of Human Services online.
